Where I can find dress shoes for women size 4.5 meters wide and super?

I have the hardest time finding dress shoes, high heels for my size 4.5 meters wide super. I tried Zappos, Nordstrom and almost any shoe store I can find. I have also large "rupture of the toe, which looks awful with most pumps or peep toe slingbacks. Please help!
